A Brief Overview

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.

 

What You Will Do

  • Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
         • Provides age specific care to patients/families in all age groups.
  • Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs. 
         • Develops plans with attention to individual patient and family goals
         • Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
  • Communicates/collaborates with the work team using a multidisciplinary approach.
  • Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
  • Assists with recruitment, and orientation/mentoring/education of new staff and or social work interns.

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
 Qualifications 

Education

  • Master's Degree from an accredited school of Social Work (Required)

 

Work Experience

  • Hospital/Health Agency experience. (Preferred)

 

Knowledge, Skills, & Abilities

  • Computer skills (Required proficiency)
  • Theoretical and clinical knowledge regarding health care delivery (Required proficiency)

Licenses and Certifications

  • Licensed Social Worker (LSW) (Required)
  • Licensed Independent Social Worker (LISW) (Preferred within one year of eligibility)

 

Position Details/Preferred Qualifications

Post-Master’s experience is strongly preferred. Previous experience with pediatrics and/or OB is also preferred. This is an outpatient position located at our Rainbow Ahuja Center for Women and Children (Midtown). The role of the social worker is critical in order to meet the psychosocial needs of the population that visits the clinic. The social worker completes psychosocial assessments, provides supportive counseling, and develops interventions to meet the needs of the patients. They see patients/families in the pediatric clinic where children come for primary care and in the women’s health clinic where patients receive prenatal care. Psychosocial issues that are identified by the provider are then referred to the social worker for: assessment, intervention, supportive counseling, and resource referrals. Examples of issues may include domestic violence, child abuse, poverty, depression, homelessness, behavior problems, teen pregnancy, etc. Schedule is Monday-Friday, days.
