What You'll Be Doing
- Provide skilled home health nursing visits throughout multiple service areas
- Perform comprehensive patient assessments
- Deliver wound care , medication management, injections, and other skilled nursing services
- Educate patients and caregivers on diagnoses and treatment plans
- Collaborate with physicians, therapists, and interdisciplinary care teams
- Float between territories to support census growth, vacations, and staffing needs
- Complete timely documentation using Homecare Homebase (HCHB)
- Help ensure continuity of care for patients across the organization
Who We're Looking For
- Active Minnesota Registered Nurse (RN) license
- Experience in home health , hospital, rehabilitation, skilled nursing, or acute care
- Strong clinical judgment and assessment skills
- Comfortable adapting to different territories and patient populations
- Excellent communication and organizational abilities
- Reliable transportation and willingness to travel throughout the Twin Cities Metro
