Key Responsibilities:
- Unit Leadership: Lead and empower our CNA team, fostering a collaborative environment where every resident receives premier care.
- Clinical Vigilance: Conduct sharp-eyed assessments and document evaluations, acting as the first line of defense in identifying and addressing changes in resident condition.
- Patient-Centered Planning: Collaborate on personalized recovery goals that help our residents reclaim their independence and return home stronger.
- Interdisciplinary Synergy: Partner closely with RNs, physicians, and therapists to deliver a holistic, "whole-person" care experience.
- Precision Nursing: Manage complex treatments, medications, and clinical procedures with accuracy and a high standard of safety.
- License: Current, unrestricted Michigan LPN License.
- Character: A champion for resident dignity who leads with integrity and empathy.
- Tech-Savvy: Comfortable navigating Electronic Health Records (EHR) and digital clinical documentation.
- Team Player: A natural communicator who knows how to support and motivate a care team.
- Background: 1+ year of experience in a Skilled Nursing (SNF) or Long-Term Care (LTC) setting (New grads are also encouraged to apply!).
- Specialty Knowledge: A solid grasp of modern infection control and best practices.
