The Adult Care Assistant provides personal care, companionship, transportation assistance, and support services to participants under the direction of the interdisciplinary care team. Responsibilities include assisting with activities of daily living, meal preparation, housekeeping, documentation, and transportation services.

 

Patient Care

  • Record pertinent information related to participant care and services provided.
  • Assist participants with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, transfers, ambulation, and personal hygiene.
  • Assist with catheter care, skin care, incontinence care, and obtaining vital signs as directed.
  • Remind participants to take medications according to the care plan.
  • Perform meal-related functions, including meal preparation, serving meals and snacks, assisting with feeding when necessary, and following dietary and food storage guidelines.
  • Assist participants with therapy exercises under the direction of Physical and Occupational Therapy staff.
  • Observe, document, and report changes in participant condition, incidents, and other concerns to the appropriate team members.
  • Provide companionship, social interaction, and emotional support to participants.
  • Accompany participants to medical appointments and community activities as outlined in the care plan.
  • Assist with grocery shopping and other approved errands independently or with the participant.
  • Maintain a safe, clean, and healthy environment through light housekeeping, laundry, linen changes, and routine household tasks.
  • Follow Participant Rights guidelines at all times, ensuring participants are treated with dignity, respect, and confidentiality.
  • Communicate effectively and professionally with participants, families, staff, and community partners.
  • Accurately prepare daily records and documentation and submit all required paperwork to the office by established deadlines.
  • Participate in mandatory in-service education, training programs, and staff development activities.
  • Perform other duties as assigned by the supervisor.

Quality

  • Demonstrate knowledge of and participation in unit-based quality improvement initiatives.
  • Document participant care daily in the Electronic Health Record (EHR) and/or appropriate home care documentation system.
  • Maintain accurate, timely, and complete participant records in accordance with organizational policies and regulatory requirements.

Collaboration & Partnership

  • Work collaboratively with the Program of All-Inclusive Care for the Elderly (PACE) Interdisciplinary Team to ensure participants receive coordinated, high-quality care.
  • Follow care plans, policies, procedures, and participant care instructions established by the PACE Interdisciplinary Team.
  • Communicate effectively with participants, families, caregivers, and interdisciplinary team members to support positive participant outcomes.
  • Demonstrate professionalism and a commitment to participant-centered care while contributing to a collaborative team environment.

Other Duties as Assigned

  • Complete additional duties and responsibilities as assigned by leadership.
  • Actively participate in unit teams, work groups, committees, and organizational initiatives as requested.
  • Demonstrate a team-oriented approach by anticipating needs and voluntarily assisting coworkers when appropriate.
  • Attend and participate in all mandatory in-services and training programs, including those required by CMS regulations.

 

 

Required:

  • High School Diploma or GED.
  • Current Louisiana CNA Certification.
  • Current BLS Certification.
  • Minimum one (1) year of CNA or Personal Care experience working with elderly or frail adults.
  • Completion of a CNA training program focused on the care of older adults.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Ability to successfully complete a one-way Spark Hire video interview.
  • Education, licensure, certification, and employment history must successfully pass verification through an approved third-party vendor.

Driver Requirements:

  • Valid Louisiana Driver's License required.
  • Clean driving record with no major violations.
  • Must meet all FMOLHS-approved driver qualifications.
  • Ability to safely transport participants in accordance with organizational policies.

Additional Hiring Requirements:

The mission of PACE Baton Rouge is to enable the aging population to live with dignity in their communities. With PACE Baton Rouge's assistance, participants can remain in the secure surroundings of their own homes and communities while providing family members and caregivers with much-needed professional guidance, physical relief and emotional support.

PACE Baton Rouge takes a team approach to care with a diverse, multi-disciplinary team. The team works together to develop individual care plans and goals and carefully monitor progress.
